Physical Exam Requirements
for Munster High School and Wilbur Wright Middle School 

6^th or 9^th grade students not planning to try out for a school athletic team:  
1.  MHS and WWMS Physical Examination form only. 

^6th or 9^th grade student planning to try out for a school athletic team:
            1.  MHS and WWMS Physical Examination Form
            2.  IHSAA Pre-Participation Physical Evaluation Form
            3.  Athlete’s Medical and Injury Information 

Students new to MHS or WWMS in other grades fall under the same requirements as 6^th and 9^th grade students. 

Returning students in grades other than 6^th or 9^th that are planning to try out for a school athletic team:
            1.  IHSAA Pre-Participation Examination Form
            2.  Athlete’s Medical and Injury Information
