SCHOOL TOWN OF MUNSTER ADMINISTERING MEDICATION DURING SCHOOL HOURS

Prescribed medication or medically prescribed treatments should be administered during school hours only when failure to do so would jeopardize the health of the student, the student would not be able to attend school if the medication or treatment were not made available during school hours, or the child is disabled and requires medication to benefit from his/her educational program. Medication will be administered by a staff member designated by the principal (usually the school nurse).

The form for Authorization for Prescription Drugs and/or Treatments must be filled out by the doctor and parent. If the medication is to be carried by the student because of the emergent nature of the student's condition and treatment (inhalers, epipens, etc.), the bottom of the same form must be filled out by the doctor, parent, and student.

For each prescribed medication the container shall have a pharmacist's label with the following information:

1. student's name

2. physician's name

3. date

4. pharmacy name and telephone

5. name of medication

6. prescribed dosage and frequency

7. special handling and storage directions

All medication is stored in a locked cabinet in the nurse's office. NO STUDENT IS PERMITTED TO HAVE ANY PRESCRIPTION MEDICATION IN THEIR POSSESSION DURING THE SCHOOL DAY UNLESS AUTHORIZED IN WRITING BY HIS/HER PHYSICIAN

Non-prescription medication may be given by designated staff upon receipt of proper paperwork and the medication in the original bottle. A student in grades 9-12 may be allowed to possess and self-administer an over-the-counter medication upon the written authorization of the parent only if the parent has completed the proper paperwork and submitted it to the Health Service to be filed in the student's record.

Medication dosage: Prescription medication at no time will be given in a dosage that exceeds that listed in the Physician's Desk Reference. In the case of over-the-counter medication, medication will not be given in a dosage that exceeds the medication instructions on the package unless authorized by the physician.

ALL MEDICATION AND TREATMENT PAPERWORK MUST BE RENEWED EACH YEAR.
